Why not get a 11.99€ barematel at scaleway that has 2.5gbps unmetered internal bandwidth, upgrade it to 1TB SSD (+19€/m extra) and then use ~5 200mbps vps at 3€/m to stream the files?
That would come down to ~50€/m based on how many servers you actually need.
Keep in mind that those scaleway ssds are network-attached. Means you won't get 2.5gbit internal bandwidth when serving files from those, but more like 1.25gbit (best case).
Better go for their direct-ssd thingy (or more than one), and spread your data a bit.
The load balancer will become the weakest link in the chain. The traffic will be limited by its throughput.

So I have contacted WorldStream. They offer 1Gbit for free. The rest is a play on words. 10Gbit means I'm on a 10Gbit port but only 2Gbps is offered. A 10Gbit speed is € 2250 per month.
Let us know if you find either 2 or 10 Gb - good quality! - pipes cheaper anywhere.
I think at this point the solution is horizontal scaling over different VPS networks. I'll see what I can do with that.

That said, I'd still stick to dedis. Worldstream has rather attractive offers where you can spread, say, another 250TB over a couple of those for a quite nice total cost, getting resilience for free along the way, and you can also spread over different locations, somewhat adapting to your clients.
As for coordination, that's not a big problem. Can be done by a pro in little time.
You could run multiple servers and use Cloudflare as a load balancer. They have a closed trial for a load balancing feature.
In the mean time, multiple A records can acts as simple round-robin balancing. That should work OK with similar servers, when combined with your own health checks and API calls to add/remove records if those severs change.

CloudFlare load-balancing will probably be pretty costly.

Get in touch with Openload and work out a deal? They can exempt you from DMCA removals if your site gets high traffic.
If you want something self-hosted I suggest you get in touch with sia.tech developers. They may take money upfront for developing the streaming service but it wounds up a lot cheaper in the end.

Not if you treat your content as CDN-served. Then your "loadbalancer" becomes a fancy "what server is this file on? Oh, here's a link mr Client" - box. (which is technically not a loadbalancer in the purest sense of the word)
That way, you can do some beefy-ish server (a metered gbit dedi would do fine) with a bunch of cheap unmetered 100mbit-1gbit slave servers. Have all your content on the main box, rsync A-C to slave1, D-F to slave2, G-I to slave3. Then all you have to do is a quick lookup for where the content is at.
When a slave dies, you can easily keep serving the (missing) content from the main box while setting up a new slave and syncing it, or even have slaves with duplicated content. This will also provide you with backups of a sort.
No need. I'm already almost done recoding the entire thing. It's using Golang, so I'm not too worried about the future scalability or speed.
What I should do is simply have an origin on Gigabit and use Nginx on edge w/ proxy_cache. I own an nginx+ license so purging, draining and routing through the API will be a breeze.
I'll then use a deploy script or a docker image on multiple digital ocean VPSes, calculate the current load through the Nginx+ API, create a small predictive algorithm connected through Google Analytics to predict peak times. Then get the droplets IPs, set their TTL to 5 mins or none, add it all on CloudFlare using the API, and I'll be done.
