Maybe you can add an image upload function for a screenshot. (Or a link to an image upload service). Or a link to pastebay for config files and such. I know you can type it yourself, but a "If you have any errors with config files, please upload them here" is just nice. Same goes for the screenshots...
That design is simple, but I like it. Very neat.
