If you don't want to spend a lot of money on something like Grasshopper or Zendesk phone, maybe TollFreeForwarding.com + your own cell phone. You'll get a nice fancy IVR Menu setup etc... and not spend too much.

Haven't given that much thought on it. A provider that can add numbers from different countries is a plus. That would open up for local calls no matter from where the caller is located.
toll-free numbers are a nice feature but it will probably not work if the caller is not from the same country.
If I have to chose only one country I would prefer Sweden since thats where I live.

try didlogic, used them since 2013 and no issue.
I would recommend http://www.voicebuy.com and http://www.voxbeam.com they are both in essence wholesale providers but you can use them as an individual with $50 topups. Voxbeam is ran by the same corporation as Localphone. Both have very low rates and multiple rate tiers (silver, gold, premium, etc.)
Also would add http://www.anveo.com
Anveo allows you to build a pretty complex system, and you pay per usage of building blocks. Many countries for incoming numbers.
I was looking at this (a few months back) and remember thinking that twilio would be the best idea. Though I prefer fixed pricing in most products (no surprises), I didn't think I would get large call volumes and will not end up spending more than 10$ a month.
Were you correct in your assumptions? Was Twilio as cheap as you thought?
Sorry, didn't have a chance to use it for long. We switched to Zendesk (they had a 99$/year promo) and it helped us setup a KB and chat integration into the website.
PS: Was just going through the Zendesk compare page, they use Twilio infrastructure for their talk platform.
The phone number can't call outbound without violating the "free" you said.
With Twilio, you need PHP programming skills to make use of it.
They offer a SIP trunk so php knowledge shouldn't be needed.
To be fair, being able to interface SIP is generally beyond most PHP programmers.
