Minimum Specifications :

- 15 GB SSD / SAN ( 25 Gb Preferred )
- 2 x IPv4 ( One for SSL )
- 1 TB Bandwidth
- Good CPU for Direct Admin Install and Run
Required Locations :
- 2 x Europe
- 1 x Canada , Preferably OVH DC
- 2 x US
Optional :
- DDoS Protection ( Minimum 10 Gbps , Dont need but would Consider )
Budget : For Each VPS i have a budget of $4 - $7 /Mo ( Or $40 /Yr ) , Lower the Better

Just going to ignore your budget because it is not realistic, but DO+Leaseweb+OVH covers all of your needs.
Double your budget and your good to go or drop the SSD SAN request and your good to go.
OK. Then take a look at http://www.ultravps.com/ssd-vps.php as for US. KVM node based on Xeon E5-1660v3, DDR4-2133, SSD RAID-10. Port speed is 100Mbps, 2 IPv4 included. In order to get VPS-1 plan for $26/year you can use MAR60OFFAN promo code.
But I would rather Dediserve or Vultr (768MB RAM SSD plan for $5/m + $2/m additiona IP) as for US location since I have never used UltraVPS personally. However UltraVPS is a subsidiary of Versaweb so they are probably trusted.
Runabove for CA and EU, as was mentioned above.
