high speed bw until limit
then real unlimited bw until quota reset with low speed, do not be afraid to make this really slow (i'd say 1gbit is too much) since I expect this to be 100% unlimited
BUT, ability to buy more high speed bw that's not overpriced like 10x but actually be reasonable
and then probably account settings to automatically buy more (or post-billing, your choice, easier when it comes to usage based costs, but higher risk for you if they run away without paying) high speed bw for enterprise users who prefer to spend money than to have their shit go down (not sure how many of these users you have, but if you'd like them you probably need this), in the best of cases with a hard-set limit for the middle tier of users

I prefer high port speed until reach the limit, and limitation (100-200 mbit) until monthly reset
To buy more BW it can be an option for those who really need high BW all the time. But those who jsut need something for few days can have that limited option.
The service suspended until reset it's not an option.

I don't tend to use much bandwidth, but I absolutely hate the concept of my server being suspended if I ever I hit the limit so at that point a limited connection speed is a must, (even if it's only 5Mbps or whatever).
