I think (I might be wrong) self-hosted Simple Login tries to send email from the hosted instance so it's like hosting your own email server which is pain.
I need to find a way to use MXRoute together with Simple Login, where I receive and send mails using MXRoute but create / delete aliases using Simple Login.

Yeah. The five eye things doesn’t bother me too much. More about if I trust the company to hang around and run a reliable service, which outweighs concerns I might have of government snooping. Noting I’m Australian and Fastmail are as well. I’m using so many other US or AU based services anyway, and before I used Fastmail I was using Google’s services which are probably no better from that perspective (although I felt OK there too given it was a paid Workspace account).
I went with Fastmail as the webmail was clean, service performed well and things like push mail to iOS just worked natively without needing to use a vendor specific app. I did “trial” them with a secondary account for about a year before fully migrating over. Took my time as Google indicated with quite an extended period concerning the service changes they were making which pushed me away from them after 10-15 years.
But for those concerned about it, there’s alternatives I see mentioned like mailbox.org etc. Proton’s an obvious one which I’ve considered a few times, but while I think their offering looks good in many respects, Ive always been put off by the fact to achieve that piracy I’m forced to use their apps (on mobile anyway) and would in particular give up OS integration with my iOS device. Just not prepared to do that, but many people are more than happy. Their clients not too bad all things considered but.
Whatever your choice, I vote to use a custom domain but.
