Looking for a NAT IPv4 VPS with domain forwarding

Is there any NAT VPS provider which supports domain forwarding, I searched online but could not find any, gullo, natvps.net also stopped domain forwarding.
Domain forwarding 80/443
Nice to have:
/64 IPv6 would be nice
US east or central.
Bandwidth: 100MB - 1GB a month.
Need it for IPv4 to IPv6 proxy for occasional access of home server.

Depending on how cheap you are looking to go, we have plans with public ipv4/ipv6 located in NY starting at $15/year. smcvps.com
Gotta be NAT IPv6 + routed /64, to reduce our IPv4 footprint.
Cloudflare origin rules kinda work......
