You can't pick best at that price, there's the MassiGrid offer with questionable lifetime. Or come and sit around the megathread to catch something similar from more reputable providers.

As others have said, the IP address costs more than you want to pay. Some providers own IP space and and can afford $12/yr, hoping that you upgrade or buy more VPS at full price.
There is no flat rate costs for IPv4 addresses, it all depends on how much they bought the block for/how much they're leasing it for.
Here's a general example on how much IPv4 addresses cost (This was the first result on google for ipv4 address leasing, I don't know if this is standard pricing or not but it should certainly still be profitable to run a server for 12/y. It just depends if you really want a profit margin of 5 cents.)
