SolusVM Control Panel has all that information and some monitoring. You can scale to hundreds of servers and thousands of VPS's.
Zabbix for full monitoring and sending alerts.

Same here, I have a personal wiki with details of all my servers.
DNS hostnames e.g us1.server.domain.com or us1.purpose.server.com
combined with an entry in the monitoring server (Zabbix) complete with a name "US Chicago Fallback Server - X4B"
A shortcut on the desktop "US1 - X4B" to putty with a ssh key
Presently have scaled this method to around 20 servers.
Zabbix + external services like Uptimerobot for 'rough' monitoring

Heck yea! I have observium setup and use Chef to send out my SNMP configs to all of my servers. I'm only monitoring 15 servers but it works well.
I'm a simple person. I use a spreadsheet that I manually update.
