Any hosts that offer IPs with a one time setup fee? With OpenVZ and Centos 5 x64?
OVH, end of the list.
IPs always have an associated annual host, even if the provider buys legacy space, so I wouldn't trust anyone advertising a "one-time fee".

Do Ovh pay to lir or ripe for the ips yearly or only once?
Both - OVH buys legacy space which is paid with their RIPE account (around 1500EUR regardless of prefixes/size)
