Why were they even given a provider tag?

We cannot see the future. They are explicitly told to stay within pricing limits during the application process. He even waited the required 14 days after the initial ticket before asking for the tag to be placed on his account.
Since this is the second violation, and he was reminded about the rules, I have revoked the tag and permanently banned them. If a provider cannot obey the rules after being warned once and reminded on the ticket, they don't deserve to be here in the first place.
