Cheapest dedicated server with /29 subnet (or higher) ?

i need cheap dedicated server with /29 subnet (5 usable IPs in that subnet) with ability to buy larger subnets like maybe even /27 subnet (29 usable IPs). I would like to be able to set rDNS for the IPs.
I do not care about server location much, the requirement would be decent CPU supporting virtualization (will be used for XEN or KVM), at least 4GB RAM, 120GB HDD (preferrably raid for redundancy like raid 1), price for the server like $15/month, /29 subnet like $5/month.

Probably not going to happen, but there are some choices there if you don't need hardware virtualization support.
