http://www.budgetvz.com/ would have everything you need, and its only about 5 bucks for a whole year. If you don't know how to set it up, give me a PM and Ill install it for you. Please note I do not work for them btw.
Why would you need a dedicated ipv4? You are using it as a VPN so ports aren't necessary (aka nat ipv4 vps will be fine), UNLESS if you want to host something on that same box and use any port you want.
So if you want the absolute cheapest server just for VPN to browse any site without restrictions, then any of those LES nat ipv4 vps will do.
OR budgetvz.com if you must have a dedicated ipv4.

You are already aware of Egg that was restocked by @jmckeag12 yesterday in my book.
What @bersey said seems good. I've been using a RamNode VPS for US$15 per year. It's a bit more than the other low enders, but they also have good peering.
Because I use it to access my vps' and ftp servers from my college network that blocks ongoing ports unless the usual ones like 80 443 so I can't set the VPN on a random port provided by nat ipv4.
OK I see what you mean. Then yea budgetvz and moosevz would be good and cheap, depends on which location you need/want.
