OMG! Have you looked at the thread? The issue do happen when the complete servers is forcefully shutdown (like a node failure, power outtake etc etc). So it MAY not have nothing to do with just MySQL going down.
I have only seen it when the complete server has gone down. (or I force a shutdown on my cloud servers)
