well, there are many providers that offer /64 ipv6 (buyvm, dedipath, etc), but i dont know if you can do the proxies thing.
Virmarch offers proxies but are in ipv4, you can check with them.

This is something we do as long as you can meet our certain requirements.
Looking specifically for IPv6 as those are cheap than IPv4.
Its better to get it from a provider who have experience in this field. I am sure its few minutes work for anyone who deals with vps providing business. IPv4 i can ownself setup using squid. Just never dealt with IPv6 before. And getting it setup from the provider is way better than opening 100s of support tickets.
And yeah some ice cream wont harm either.
