I am pretty sure it just gets you a link direct from Microsoft's servers.
It's pretty telling when Windows has tons of built in drivers these days and yet won't include virtio.
Windows Server 2012 :

They are all VirtIO included. The author claimed that they are official ISO provided by MS and the only thing he modified was adding the VirtIO driver, but you should also be careful.
I am looking for Win Server 2022.
Some providers (for example liteserver, inceptionhosting) allow you to mount 2 ISO all together. So yeah that can help.

You don't even need two isos mounted at once. At the point in Windows setup where it asks for the disk driver you can safely remove the Windows iso since the environment is in a ramdisk at that point. You then plug in the virtio iso, load the driver and put the Windows iso back afterwards. This method is fully supported by Microsoft and was used back in the day when drivers were on CDs and computers still had CD drives.
As long as you are able to mount a virtio iso while in operation, there is no need to integrate the drivers aside from convenience. In some special cases this convenience gain could be large though, which is fair
Yes. That's just downloading UUP files from Windows Update then compile it into a fully working ISO
