How to set this up? about SMTP

I mean,I know what SMTP host is , and also username/port/SSL/passwd,
I filled it correctly,but mails wont send out.
I dont know what is "sender" and "name",should I fill them up? what should I fill in?

Name is the name for the email account eg Forum Notifications where Sender is the email itself.
