One of the cheapest options in Russia is FirstByte (Moscow DataPro DC). Check their KVM SAS and SSD line. Custom ISO is supported (in Russian).

No, you don't need that. If a provider supports any linux and or BSD image you can install any other. Also not that many providers offer quite powerful rescue systems that can be abused to quite comfortably install other distros.

Win for $6 License included. Russian providers all offer Win already licensed. There no trials or own.
